Once you submit this form, our secretary will initiate your FFF license request. You will then receive an email from the FFF titled 'Licence FFF'. Please follow the instructions in that email to upload your identity document and medical certificate directly to the secure FFF portal.
Please watch your inbox for an automated email from the FFF (check your Spam/Junk folder if you don't see it). This email will contain a secure link for you to finalize your file directly, including uploading your photo and confirming your health information."
